1. Website Use

2. Appointments and Services
Appointment requests submitted through our website or patient portal are subject to confirmation by Story Dental.
We reserve the right to reschedule or cancel appointments as needed.
Dental treatment recommendations are based on clinical evaluation and professional judgment.

3. Payment Terms
Payment is due at the time services are rendered unless prior arrangements have been made.
We accept major credit cards, debit cards, cash, and other approved payment methods.
If a balance remains unpaid beyond 30 days, a late fee of $25 or 10% of the outstanding balance (whichever is greater) may be applied once per outstanding balance.
Patients are responsible for any fees not covered by insurance.

4. Insurance
As a courtesy, we may submit claims to your insurance provider. However, you remain responsible for the full balance of services provided.
Insurance estimates are not guarantees of payment.

5. SMS Messaging Program
Story Dental may send SMS messages related to:

Appointment confirmations and reminders
Reschedule notifications
Account updates
Patient portal alerts
Customer support communications
Verification codes

Message frequency varies depending on appointments and account activity.
Message and data rates may apply.
Opting Out: You can cancel the SMS service at any time by replying STOP to any message. After you send STOP, we will send a final confirmation message to confirm you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you wish to re-enroll, you may sign up again through our SMS consent form at https://www.storydental.com/sms-consent.
